The Readmission process has recently changed. Forms are no longer needed. You must meet with a Counselor to make a Readmission Ed Plan, and you must complete the Back on Track Dismissal Workshop in your portal. The Workshop can be found in the Online Classes and Workshops block, under the Workshops heading. Once you have completed this workshop contact the Counseling office on the Santa Rosa (707) 527-4451 or Petaluma (707) 778-3914 campus to speak with a Counselor to make the Readmission ed plan.
